Three Facial Plastic Surgery Procedures You’ve Never Heard Of

You thought you knew everything about facial plastic surgery and cutting-edge aesthetic procedures. And maybe you do! But if you don’t, the following are a few of the lesser-known surgical aesthetic enhancements that I perform for women and men who want to boost their physical appearance as well as their self-confidence. As a double-board certified facial plastic surgeon in New York City who also trained as an artist, my goal is to make you look like the very best version of yourself and increase your Aesthetic Confidence®:

Buccal fat removal

before and after of buccal fat removalYour “chipmunk cheeks” were adorable when you were five, but now you want a sophisticated, sleek look that matches your personality and your level of achievement. By removing excess buccal (i.e. cheek) fat and tissue, I create a slender lower face that emphasizes and dramatizes your cheekbones.

The entire outpatient buccal fat removal surgery only takes about an hour and can be done under general or local anesthesia. During this procedure, I make a small incision inside both cheeks, so you won’t have a visible scar. With care and precision, I sculpt away excess fat to create cheeks that are lean and beautifully contoured.

The incisions are then closed with dissolvable sutures. You can return to work or regular activities as soon as the next day.

G.I Jaw®

before and after photo of a man who underwent the gi jaw procedure in new yorkIf you’re a man, you may envy men like Brad Pitt or even action figures — like G.I. Joe who I named this procedure after — who have that classically sculpted strong jawline. A square, chiseled, defined jaw conveys strength, dominance, and success. While you may not have been born with the bone structure that exudes the kind of confidence you want to present to the world, a G.I. Jaw procedure helps you look as masculine and attractive as you want to be.

Looking to achieve a strong jawline through a cosmetic procedure has become so popular that according to the American Academy of Facial Plastic and Reconstructive Surgery, general plastic surgeries are on the rise. But my patented G.I. Jaw procedure is more than simply inserting a medical-grade jaw implant or building the jaw non-surgically with long-lasting dermal fillers. I may also recommend other changes to create a more masculine appearance, which keeps your new jawline in harmony with your other features. Recommendations may include:

My aim is to create improvements that convey more strength and masculinity, while still looking natural and like yourself.

Inserting medical-grade implants is a minimally invasive procedure that can be done in the office under local anesthesia. You may experience some swelling or discomfort afterward. Generally, most men don’t have discomfort following a nonsurgical G.I. Jaw procedure performed with dermal fillers.

Micro-Rhinoplasty

before and after of a owmna who received a mirco rhinoplasty in new york

Do you want to improve the shape of your nose, but the idea of undergoing a full rhinoplasty — which requires breaking the nasal bone and cartilage and a healing period of up to one year — just doesn’t appeal to you? If you only want to remove bumps and lumps on your nose, I can perform a micro-rhinoplasty that requires just one tiny incision in your nostril, no bone breakage, and no significant downtime.

Using specialized tools, I insert a high-tech, air-powered micro-rasp into the incision and gently reshape your nasal bone and cartilage, sculpting away humps and bumps. The end result is a nose that’s straighter, more aesthetically pleasing, and in better harmony with the rest of your facial features.

Unlike traditional rhinoplasty, which is performed under general anesthesia and may take several hours, micro-rhinoplasty is done in my Manhattan office under local anesthesia. The entire procedure only takes about  5-15 minutes.

Your recovery time is also cut down. You’re swollen for a few days after your procedure but can return to most of your normal (non-strenuous) activities within 1-2 days.

Why Requesting A Celebrity’s Nose Isn’t Always Best for Rhinoplasty Goals

Rhinoplasty before and after new yorkA rhinoplasty, more commonly known as a nose job in New York, is treated by most people as a simple act of shopping –that is, purchasing a new nose. I know it sounds easy and straightforward.  It begins by you wanting to correct an issue with your nose. So, you browse through social media and “shop” for the celebrity nose you like, then set an appointment with your surgeon.

Simple, right? Not really. Whenever you plan on undergoing invasive cosmetic changes, there is one crucial factor that must be addressed: your EXPECTATIONS for your surgery. In this blog, I share some of my tips for managing your expectations about getting a nose job and one crucial element for getting the nose you will love.

Only Use Celebrity Noses As Inspiration

During a rhinoplasty consultation, I ask my patients to thoroughly discuss their desired results. This way, we can share a Unified Vision. A Unified Vision incorporates the patient’s aesthetic goals along with what I deem is surgically feasible with their given anatomy. Often times when we find certain individuals attractive, we naturally go about wanting to emulate their facial features. Celebrities under the spotlight often have variants of a slim, dainty nose that is turned-up, a.k.a. the celestial or “button nose.” My patients often bring in photos of celebrities; Jennifer Lopez, Kim Kardashian-West, Scarlett Johansson, Angelina Jolie, and Meghan Markle are amongst the most popular. Patients like to come prepared and bring in these photos as a point of reference for how they’d like their noses to turn out. I prefer my patients to bring in these photos because I use them as an effective communication tool to determine what the patient desires, as well as to see how realistic their expectations are.

The Goal of Rhinoplasty

I explain to my patients that the goal of a rhinoplasty procedure is to reshape the nose to create an overall improvement to the facial structure and aesthetic. In other words, a nose job can help you improve the balance of your facial features which ultimately will increase your self-confidence. But don’t expect that someone else’s nose will look right for your face. It should not be your goal to match somebody else’s appearance, but only take them as a source of inspiration. Everybody is unique in their facial features and therefore, should also be treated as such. I help my patients create a natural-looking nose that will complement their features and honor their ethnicity, rather than give them a “cookie-cutter nose” from a celebrity.

Managing Realistic Expectations

A man on the left is shown with a larger nose. On the right the same man is shown after rhinoplasty with a smaller nose. There is no such thing as a perfect nose because “perfect” has a fluid and subjective definition. It will depend on the patient’s facial structure, size, and balance. Facial plastic surgeons can produce amazing results through modern rhinoplasty techniques, but there are always limitations. Going back to having a celebrity nose –if you specifically want to have a Meghan Markle nose, you’d probably end up having to alter your entire facial structure instead of just your nose. It is very important to have an open mind during your consultation so you can listen to your surgeon’s recommendations on what the “perfect” nose is for YOU. By understanding the procedure and having a more thoughtful and practical approach to your results, you will be giving yourself a better chance of having complete satisfaction with your rhinoplasty outcome.

Consult With A Recognized Expert In Facial Plastic Surgery

Your realistic expectations for your nose job will serve as the foundation for your cosmetic surgery. And as the surgeon, I will become the foundation of its success. Rhinoplasty is a complex surgical procedure that requires experience, expert knowledge, and top-notch skills. To set yourself up for a successful journey, begin by choosing the best facial plastic surgeon. You can review the criteria here.

I am double board-certified in Facial Plastic and Reconstructive Surgery, as well as Otolaryngology/Head and Neck Surgery, and am considered one of the country’s leading experts in facial rejuvenation and rhinoplasty. I have lectured on innovative rhinoplasty techniques at the 10th International Symposium of Facial Plastic Surgery, and consistently since 2007, have been voted among the “Best Doctors in America” and “Top Doctor of New York.” It is my goal to help my patients find a look that makes them feel beautiful and confident. I combine my expertise, experience, skill, and artistic eye to deliver those life-changing results.

Cost of Rhinoplasty

On the right, a woman is shown in profile with a bump on her nose. On the left, the same woman is shown after rhinoplasty with the bump removed. You’re ready to reshape your nose and light up your entire face with an expertly performed rhinoplasty. You want to make sure you get the most flattering, natural-looking results possible. But you’re also concerned about the cost. Is it time to search through Groupon deals, or book a trip to another country for bargain-basement surgery? While we understand wanting to save money, there is a time and a place for choosing a service based on the lowest price and surgery for your face isn’t one of them.

If you’re truly savvy about your finances, you already know that so-called “bargains” can ultimately cost more than quality goods. And that’s especially true when it comes to rhinoplasty, which is one of the most complex cosmetic facial plastic surgeries of all. Because your nose is so central to your face, you can’t hide the mistakes or hope that no one notices them. You need to get it right the first time, to save yourself both money and emotional distress. Not only that, but the nose is a functioning organ and it’s imperative that it still functions well after surgery.

Philip J. Miller, MD, FACS is a double-board certified facial plastic surgeon in New York City who specializes in rhinoplasty. He’s lectured on innovative rhinoplasty techniques at the International Symposium of Facial Plastic Surgery, written chapters on rhinoplasty for medical textbooks, and specializes in his patented NatraNose® approach to rhinoplasty procedures. Here he outlines the true costs of rhinoplasty to help you make a decision about how and where to get the most flattering nose improvements possible:

Primary Rhinoplasty

A man on the left is shown with a larger nose. On the right the same man is shown after rhinoplasty with a smaller nose.Your first nose job is considered a primary rhinoplasty, which can run anywhere from $3,500 to $25,000, depending on your location and the expertise of your surgeon. You might also opt for “tip plasty” which is less expensive but only improves the look, angle, or shape of the tip of your nose.

The nose is a highly complex, finely sculpted structure that consists of a variety of tissue types, including skin, cartilage, and bone. Lack of expertise in working with any of these mediums can result in a nose that heals incorrectly and calls unwanted attention to itself. However, a beautifully executed rhinoplasty corrects defects in the size, shape, and angle of your nose.

With an expert facial plastic surgeon such as Dr. Miller, you can expect aesthetic improvements such as:

  • Shorter nose
  • Straighter nose
  • No bumps
  • More symmetrical nostrils
  • Thinner or wider nostrils
  • Thinner nose
  • Larger nose
  • More beautifully defined tip
  • Nose that harmonizes with the rest of your face

Ideally, your primary rhinoplasty is so successful that you never need revision rhinoplasties, and, after your healing period of up to one year, your new nose is everything you ever dreamed it would be. In reality, if you choose the wrong surgeon, you could need at least one revision and potentially more. In fact, up to 10% of patients who undergo rhinoplasty will seek out a revision.

Revision Rhinoplasty

On the left, a woman is shown with an asymmetrical nose and nostrils. On the right, the same woman is shown after NatraNose with a more symmetrical noseA surgeon who is not board-certified as a facial plastic surgeon, or even a plastic surgeon who doesn’t specialize in rhinoplasty, can “botch” your nose job, leaving you with a nose that looks worse than your original nose and may cost between $3,000 to $30,000 to improve.

Keep in mind, it can take up to a full year to heal after rhinoplasty and to assess whether you need additional surgery, but common reasons for needing a revision rhinoplasty include:

  • Too much cartilage removed (collapsed nose)
  • Over-rotated tip (pig-like nose)
  • Difficulty breathing
  • Drooping tip
  • Crooked nose
  • Asymmetric nostrils
  • Nose doesn’t harmonize with facial features
  • Nose inappropriate for ethnicity or gender

If your surgeon botched your primary rhinoplasty, you could require one or even several revision rhinoplasties. When the mistake is minor and you otherwise trust your facial plastic surgeon’s expertise, you may not be charged labor for the revision or may pay a reduced price (another reason to stay close to home for surgical procedures). However, if you changed your mind about what you wanted your nose to look like, or if the mistake is so grave that you no longer trust your surgeon, you can pay anywhere from $3,000 to $30,000 or more for each revision procedure. That’s why seeking out a bargain when it comes to rhinoplasty isn’t the best choice.

As complex as a primary rhinoplasty is, revision rhinoplasty is even more challenging. Even a well-executed primary rhinoplasty creates scar tissue in your nose. A poorly done nose job could leave a lot of scar tissue, plus create structural problems that may be difficult to rectify.

If you need a revision rhinoplasty, it is imperative to find the most qualified surgeon possible — one who specializes in revision rhinoplasty. The quality of your cartilage, bone, and skin have already been compromised enough by an inexpert surgeon. You’ll now require someone who has experience working with and compensating for that structural damage. Dr. Miller is an expert at correcting mistakes made by other surgeons and giving you the nose you want and deserve.

Nonsurgical rhinoplasty

Nonsurgical rhinoplasty costs about 10 times less than a surgical nose job, with charges ranging from just $300 to about $1,800, depending on your location, the expertise of your surgeon, and the amount of filler that’s required to improve your nose. If your nose is bumpy or asymmetrical, but you’re otherwise happy with its length and shape, the most cost-effective solution could be a nonsurgical rhinoplasty accomplished with the aid of hyaluronic-acid based dermal fillers.

Dr. Miller also uses liquid rhinoplasty to reshape and reposition your nasal tip and recontour your nose. Other than some soreness and swelling directly after the injections, you won’t have any recovery time or side effects from liquid rhinoplasty. You will also see immediate results.

Even though nonsurgical rhinoplasty is much less complex than rhinoplasty surgery, it still requires knowledge about the nose’s anatomy, aesthetics, and facial harmony. The good news is, if you don’t like your nonsurgical rhinoplasty, the filler can be dissolved.

If you’re nervous about rhinoplasty or you want to make some improvements to your nose while saving up for primary rhinoplasty surgery, nonsurgical rhinoplasty with dermal fillers is a great way to ease into a nose job. Many women and men opt for a nonsurgical nose job first, as a way of “auditioning” a potential new nose shape. Others are so happy with their results that they continue touching-up their nose with fillers, without ever having to undergo surgery. However, you’ll need to have re-treatments in order to maintain your results versus a surgical procedure which provides permanent results.

Septoplasty (functional nose job)

If you have a deviated septum, your septum (the nasal bone and cartilage that divide your nose in half) veers into and takes up extra space in one of your nostrils, making it hard to breathe. Many women and men require a septoplasty in addition to their desire for cosmetic rhinoplasty. A septoplasty alone will not improve the aesthetics of the nose at all.  But, since a deviated septum is a medical condition that impairs your ability to take in full breaths through your nose, your septoplasty may be covered by your medical insurance.

When you combine rhinoplasty with septoplasty, however, only the portion of the operation that’s medically necessary is eligible for insurance coverage. Aesthetic improvements are not eligible for coverage or reimbursement; don’t trust and don’t work with the unethical surgeons who say they are or who try to manipulate the system to pay for your cosmetic surgery.

What you’re paying for?

In addition to paying for the expertise, aesthetic sense, and skill of your surgeon, rhinoplasty has a number of other expenses that must be considered when making a final decision based on price. Your procedure may vary depending on how complex and difficult it is. Dr. Miller’s quotes are always all-inclusive, and include the following:

  • Surgeon’s fee
  • Anesthesiologist’s fee
  • Anesthesia
  • Surgical center
  • Postsurgical care (follow up visits and tests)
  • Medication-related to surgery
  • Bandages and wraps

Dr. Miller has the advantage of his own surgical suite, which helps you avoid a potentially expensive hospital stay. When consulting with other facial plastic surgeons who specialize in rhinoplasty, always ask for a complete breakdown of their price, so you won’t be surprised by add-ons in the end.

When considering the total cost of your rhinoplasty, remember to factor in the cost of recovery.  You’ll need to take time off from work, and arrange for household help. For additional information, please visit our rhinoplasty and nose job guide.

The Rise of Medical Aesthetics for Men

Medical Aesthetics for Men for New York City Rhinoplasty Expert BlogHistorically, men’s concerns about their appearance have been focused on building up muscles to create a more masculine and powerful physique. Remember those comic-book and wrestling-magazine ads that made you feel insecure about being a “98-lb. weakling,” even though you were only 12?

But these days, the ubiquity of social media demands that men of all ages look selfie-ready at a moment’s notice. And the “gig” economy, which puts you in the financial ring with ever-younger competitors of both sexes — has made looking energetic, strong, and youthful more important than ever.

Most plastic surgeons see only a small fraction of male patients so have very little experience with men and their unique needs. Luckily, Dr. Miller and Gotham Plastic Surgery are unique because almost half of their patients are male.

Dr. Philip Miller, a groundbreaking, double-board certified facial plastic surgeon in New York City, knows what kinds of aesthetic improvements look natural and empowering to the male face. As the developer of unique, trademarked procedures such as NatraLook™ facelifts and G.I. Jaw™ chin and jaw enhancement for men, Dr. Miller is committed to making you feel like the absolute best version of yourself so you can achieve Aesthetic Confidence™.

Male Facial Aesthetics

No matter how successful and healthy you are, you can’t stop the inevitable march of time. A whole-foods diet, sun avoidance, and plenty of cell-enriching and muscle-building exercise can do wonders to keep you in optimal shape. Nevertheless, the passing years take their toll on your face, with signs of aging such as:

  • Jowling
  • Sagging skin
  • Hooded eyelids
  • Under-eye bags
  • Double chin
  • Soft neckline
  • Sunspots
  • Uneven skin tone

Before and After image of a man undergoing facial wrinkle removal procedures in New York City. When you’re bothered by facial aging, Dr. Miller spends time listening to your concerns, evaluating your particular facial structure, and designing a customized aesthetic improvement plan. Depending on how early you are in the aging cycle, he may be able to create all the improvements you need without a single incision or stitch.

What’s more, Dr. Miller understands the unique concerns men have about altering their appearance. Dr. Miller is an expert in men’s facial anatomy and can enhance their appearance while maintaining their masculinity.

Nonsurgical revitalization procedures include:

  • Botox® to smooth out dynamic wrinkles
  • Laser resurfacing to even out skin tone
  • Laser treatments to increase skin health
  • Dermal fillers (Juvèderm®, Restylane®) to fill in creases, under-eye trough, and non-surgical rhinoplasty
  • Fat grafting and fillers (Voluma®) to restore lost facial volume

Dr. Miller is also an expert at creating natural-looking rejuvenations via facial surgery or minimally invasive procedures, including:

  • Brow lift — surgically smoothing and lifting the forehead for a more youthful look
  • Upper blepharoplasty — eyelid surgery to resolve upper-eyelid hooding
  • Lower blepharoplasty — lower-lid surgery to remove fat pockets and resolve under-eye tear troughs
  • Chin liposuction — removes a double chin
  • Mini lift — resolves jowling and sagging of lower face
  • Facelift — resolves jowling and sagging of both midface and lower face
  • Fat grafting — your own excess fat is used to restore lost facial volume in midface, tear troughs, or temples
  • Neck lift — underlying plastysmal muscles (also known as neck bands, these are the vertical muscles that are evident when you make a grimace, clench your teeth, or carry stress in your neck region) tightened, double chin removed via liposuction, skin lifted, tightened, and trimmed
  • FaceTite™ — radiofrequency assisted liposuction (RFAL) to tighten loose skin and contour the lower face and neck
  • excel V+ — laser treatment to improve fine lines and wrinkles as well as hyperpigmentation

You don’t have to decide ahead of time which procedure is best for you. During your consultation, Dr. Miller makes his recommendations based on your anatomy, skin quality, and signs of aging. Together, you will create a Unified Vision decide which procedures best meet your goals.

Get “chiseled” via Jawline Augmentation

Before and after image of a man who has undergone jawline enhancement surgery in New York CityOne of the most-desired masculine facial features is a strongly chiseled jaw and neckline. Dr. Miller invented the G.I. Jaw™ procedure to give you a more powerful lower face, complete with square chin, a sharp jawbone, and well-defined neck. A strong jawline in men is so strongly associated with attractiveness and success that men account for more than half (54%)of the surgical jaw augmentations performed in the U.S., according to the American Society of Plastic Surgeons.

Dr. Miller custom designs the G.I. Jaw™ procedure based on the proportions of your upper face, lower face, nose, chin, neck, and jawline, as well as your degree of facial sagging and fat distribution. Depending on your needs, he may recommend:

  • Rhinoplasty to better balance your chin and jaw
  • Medical-grade chin implant to create a more projected, stronger chin
  • Medical-grade jaw implant to strengthen and sharpen jawline
  • Medical-grade cheek implants for lift and contouring
  • Liposuction to remove double chin and strengthen jawline
  • Nonsurgical augmentations with dermal fillers
  • Nonsurgical Kybella® to permanently melt away double chin

If you choose to go the nonsurgical route, Dr. Miller lets you know how frequently you’ll need touch-ups, based on the type of dermal filler used. If you’re considering jaw or chin implants, but aren’t yet sure, a nonsurgical augmentation with dermal fillers is a great way to “audition” a new look.

If You’re a Millennial, Here’s Why You Should Start Anti-Aging Treatments Now

remove forehead wrinklesYou’ve always prided yourself on being informed and getting the most value for your hard-earned dollars. So, when you notice a few wrinkles on your forehead, at the corners of your eyes, or realize your skin doesn’t look quite as brilliant and glowing as it used to, you’re not going to head to the store and pick up the latest “wrinkle cream.” You know that lotions and potions can only go so far. You want cosmetic options that produce real results.

Dr. Philip J. Miller, an expert facial plastic surgeon in New York City believes that any cosmetic product or procedure — whether surgical or non-surgical — should result in what he coins, “Aesthetic Confidence®”. Aesthetic Confidence comes from knowing that everything you do for yourself enhances your own unique natural beauty, making you look like the absolute best and healthiest version of yourself.

Prejuvenation is Prevention

“Prejuvenation” is one of the hottest trends in aesthetics, and continues to grow. “Prejuvenation” means keeping your skin young and lustrous looking by enacting effective, science-based prevention strategies to stop wrinkles from forming in the first place. It erases signs of sun damage and improves the overall quality of your skin. According to an article in New Beauty, millennials are starting on prejuvenation by age 26, compared to women aged 55 and older, who didn’t start addressing signs of aging until they were about 47.

Treating the First Signs of Aging

anti aging cheek implantsYou know that glowing skin starts with strong, healthy skin cells. That’s why you eat plenty of vegetables, fruits, proteins, and healthy fats. And why you exercise regularly and keep well hydrated. It’s also why you don’t leave the house without liberally applying a high-quality sunscreen with an SPF 30 or more.

Despite these healthy steps, the first wrinkles to appear tend to be the ones that are caused by the dynamic muscles that let you raise your brows, squint your eyes, and make other extreme expressions. The best way to stop those wrinkles from forming in the first place, or to prevent them from worsening, is with regular treatments with Botox® Cosmetic or another effective neuromodulator, such as Dysport® or Xeomin®.

According to the American Society of Plastic Surgeons, use of Botox and other neuromodulators has surged by 819% since the year 2000, with more than 7,000,000 treatments in the U.S. in 2017 alone. Botox stops the signals that trigger your muscles to move and press your skin into creases and wrinkles. Within a few days to two weeks after treatment, your muscles relax and your skin smooths out so that it’s wrinkle-free again. Not only that, but by treating them early, you help prevent them from worsening and new wrinkles from forming.

Treating the Secondary Signs of Aging

By the time you get into your thirties, you may notice that your skin isn’t as plump and luscious as it used to be, and neither are your lips. Every year, your body produces less of your skin’s building blocks, the proteins collagen and elastin. After age 40, too, you begin to lose about a teaspoon of fat per year in your face. Your skin also has less hyaluronic acid (HA), the lubricating agent that helps retain moisture, keeping your skin supple.

Prejuvenation isn’t just about knocking out wrinkles. It’s about restoring volume loss quickly and effectively using state-of-the-art dermal fillers made with HA, such as Juvèderm® and Restylane® and collagen stimulators such as Radiesse®. Dermal fillers come in a variety of thicknesses and consistencies to restore volume to your midface, smooth out creases around your mouth and nose, fill in tear troughs, restore your hands, and create nonsurgical lip augmentations.

Get a Jumpstart on Aging Gracefully

Prejuvenation also restores your skin’s strength, elasticity, and plumpness with fresh new collagen, elastin, and HA. Dr. Miller offers a variety of therapies, such as acne scar treatment, to stimulate your skin to turn over skin cells faster and build up new stores of collagen and elastin. Depending on your needs, he may recommend:

  • CO2 laser resurfacing
  • MicroPen microneedling
  • Monthly chemical peels

Aging gracefully needs a pro-active beauty regimen. Dr. Miller can help create a comprehensive routine that helps you maintain your health and radiance. To schedule an aesthetics appointment with Dr. Miller, call (646) 791-3025 or use the online form.

Restore Lost Facial Volume with Fillers or Implants

You look at your face in the mirror and you notice that something is lacking. Whether you’ve lost volume New York Wrinkle Treatment Fillersto aging, or just never had it due to your bone structure, you don’t have to be content with flat, hollow, or receding contours anymore. Whether you want to permanently alter the planes and curves of your face with implants, or “audition” a new look with temporary dermal fillers, Dr. Philip Miller, expert New York City facial plastic surgeon, can help you re-shape your jawline, cheekbones, and chin to get the beautiful, harmonious look and Aesthetic Confidence™ you’ve always dreamed of having.

Restore Lost Facial Volume

Aging and sun damage deplete key elements in your face that may result in a look that’s drawn and gaunt. Collagen and elastin — building blocks of skin that keep it plump, pliable, and elastic — diminish with age. The fat that once made your cheeks lovely and pinchable dissipates, too. Even your underlying bone structure atrophies over the years, so that your face no longer has the support it enjoyed in your youth.

In the early stages of aging, this process of volume loss can be reversed with treatments that stimulate your skin to produce more collagen and elastin, such as laser therapy and microneedling. However, if you’ve already lost volume, Dr. Miller can plump up your midface and build up sagging cheekbones with a lush filler such as Voluma®, which is made of hyaluronic acid (HA) — a substance that your body produces naturally. Dermal fillers are so effective for improving facial volume, they are the second most popular non-invasive cosmetic procedure performed each year.

But if you’re looking for a permanent option to restore lost facial volume, Dr. Miller can use fat from other areas of your body where you don’t need it (such as lower abdomen), and transfer it to your cheeks, where it helps restore the volume of the mid-face.

Create Model Cheekbones

You may have noticed that most celebrities and models have well-defined cheekbones. Cheekbones not only create an elegant, sophisticated look, but they also provide a natural lift to your lower face. Whether your cheekbones have receded over the years due to bone loss, or you’ve always been frustrated with flat cheekbones, Dr. Miller can reshape and contour your cheekbones with either high-quality implants or dermal fillers such as Voluma or Sculptra®.

Building up your cheekbones with Sculptra, Voluma, or layering both is a great way to try out a new look. Sculptra adds volume by stimulating the production of new collagen and elastin over time. Voluma does the same by instantly plumping up and re-contouring your skin. Layering both dermal fillers gives you volume that’s both instant and long-lasting — up to a year or more.

Of course, the most dramatic way to re-sculpt your cheekbones is with permanent silicone implants. Dr. Miller chooses the size, shape, and angle that perfectly matches your unique beauty and facial structure. If you’d also like to compensate for lost skin volume, Dr. Miller can infuse your skin with Voluma at the same time you receive your cheek implants for complete facial rejuvenation.

Transform with a G.I. Jaw®

Both men and women often long for the straight, clean jawline of a G.I. Joe or supermodel. Now you don’t have to be born with great bones to benefit from their effects. If your chin is receded or your jawline weak, Dr. Miller can resculpt your jaw and create a stronger chin with Sculptra and Voluma, or with permanent silicone implants. He may also suggest liposuction or Kybella® to remove excess fat and tighten the jawline skin.

Revive Your Facial Volume Today

Replenishing your skin and creating stronger, more lifted contours with either dermal fillers or facial implants is a great way to start the new year with a new you. Dr. Miller can even give you a preview of your new look via digital morphing. To set up an appointment and find out how fillers and implants can create the look of your dreams, call (646) 791-3025 or use the online form to book your consultation.

The Life-Changing Benefits of a Facelift

You’re considering a facelift but you’re wondering if it’s really worth the expense and downtime. If you’re listing the pros and cons while trying to come to a decision, a study from the Journal of the American Medicine Association (JAMA) Facial Plastic Surgery, supplies a number of new entries for your “pro” column. As it turns out, women who’ve had facelifts and other upper-face rejuvenation procedures aren’t just perceived as looking younger, but also as being more attractive, healthier, and more successful.

Dr. Phillip J. Miller, an expert facial plastic surgeon in New York City, has long promoted facelifts as one way to attain what he calls “Aesthetic Confidence®.” Dr. Miller’s patients relay that his surgeries do not just improve their appearance, but they have other life-changing benefits.

Some of these benefits include feeling happier and more confident after their surgery, and also that people react more positively to them than they did before. The following study, “Association Between Facial Rejuvenation and Observer Ratings of Youth, Attractiveness, Success, and Health” confirms his patients’ perceptions. Let’s explore each benefit so you can fully understand how, in the right hands,  life-changing a facelift can be.

The study included before-and-after photographs of twelve older women who’d either had eyelid surgery and a facelift, or eyelid surgery, a facelift, and a brow lift. Each of the 504 participants who looked at these photos in an online survey either saw the “before” or the “after” photograph from each of the twelve patients, but never saw both views.

When the researchers looked at how the observers rated all of the photos, they found that…

The “Afters” Looked Younger

On average, women were estimated  4 to 5 years younger in their “after” shots than they were in their “before” shots.

Technique plays an important role in creating this youthful result. When performing a facelift, Dr. Miller always lifts the underlying SMAS (submucosal aponeurotic system) layer, along with the skin. Lifting the SMAS both vertically and horizontally creates a more youthful, uplifted face that appears much more natural than when lifting skin alone. The re-positioned SMAS also provides better support for your skin, allowing your facelift to last longer.

The “Afters” Were More Attractive

On a scale of 1 to 100, the “before” photos were rated “50” on attractiveness. The “after” photos jumped up to 68.

Another technique that Dr. Miller utilizes to restore a youthful visage is fat grafting. Dr. Miller often recommends fat grafting in combination with a facelift to get the best outcome for facial rejuvenation. The fat grafts fill in areas of lost volume on your face by using fat you’ve donated from a place where you have excess fat, such as your abdomen. Using an artist’s eye, Dr. Miller uses fat grafts to give your face more youthful and balanced contours. Another added benefit is the fat grafts also help your facelift heal.

The “Afters” Looked Much Healthier

Using the same scale, the average of the “before” photos was just “50” on how healthy they looked. But the “after” shots gained 20 points.

Youthful vitality and health go hand in hand. To accelerate your healing and improve the quality of your skin overall, Dr. Miller recommends a skin rejuvenation procedure such as Fractora(TM) laser or microneedling in tandem with your facelift. Lasers and microneedling devices stimulate your skin to produce rejuvenating collagen. Your skin becomes thicker, stronger, smoother, and glows with health.

The “Afters” Looked More Successful

The study participants also rated that the women in the “after” pictures appeared more successful. The average “before” rating was only 50 but jumped to 61 for the “after” photos.

And, as Dr. Miller’s patients report, getting positive feedback on their appearance after a facelift or other cosmetic surgery really does give them a confidence boost. That confidence translates into more willingness to take risks in romance and on the job, leading to more success … and more confidence.

Are You an Ideal Candidate for Eyelid Surgery?

Are You an Ideal Candidate for Eyelid Surgery Blog If you have sagging upper eyelids or dark, hollow or baggy areas under your eyes, you may have tried — and discarded — a number of ineffective creams, lotions, and devices. Unfortunately, the skin around your eyes is thin and delicate, which is why it’s one of the first areas to show your age. And although medical-grade skin care with retinoids or other rejuvenating agents can hold back the earliest signs of aging, once your eye skin starts to bag or sag, surgery may be your best option.

Dr. Phillip Miller knows that deciding to undergo cosmetic eyelid surgery is a major step that takes careful consideration and research. As the inventor of  The Natralook Process™, Dr. Miller is committed to procedures that rejuvenate you while still maintaining your unique beauty and a natural, healthy appearance.

The Ideal Eyelid Surgery Candidate

Following are some tips that can help you determine the best procedure to get the results you want:

Are your upper lids sagging?

When your upper eyelids lose fat, collagen, and elastin, they start to droop and make you look tired or older than you feel. If the sagging is extreme, the excess skin can even hood your eyes, obscuring your vision.

Fat deposits in the upper lids also collect and droop with age, making your eyelids look thicker, too. Heavy, drooping lids are best remedied with a surgical procedure called upper blepharoplasty, otherwise known as an eyelift.

Dr. Miller hides eyelift incisions in the creases of your eyelids, then tightens the underlying lid muscle, which is one of the causes of your drooping lids. He removes excessive fat but leaves enough to make your eye look natural and youthful. He then tightens the eyelid skin and trims away any extra skin.

After you’ve healed from your surgery, your eyes look naturally more alert and youthful. Your incisions are so tiny that the hidden scar is barely visible.

Does your forehead look heavy or wrinkled?

Another sign of aging is a sagging brow that lowers the position of your eyebrows. If your eyelid skin also sags, you may benefit from both upper blepharoplasty and a brow lift.

During the procedure, Dr. Miller conceals the brow lift incisions in your forehead. He tightens the muscles in your forehead and eyelids. He lifts your brow skin — thereby smoothing out wrinkles — and trims away the extraneous skin. His skilled, artistic eye ensures that your eyebrows maintain a natural and youthful position.

If your eyelid skin hasn’t yet started to droop excessively, but you still notice that you don’t look as energized as you feel, Dr. Miller may suggest a forehead lift alone without or instead of upper blepharoplasty. This minimal surgery gives you the vibrant and energized look you deserve.

Do you have bags, wrinkles, or deep hollows under your eyes?

The first signs of aging under your eyes may be fine wrinkles, or just a darkening of your skin as it thins out due to aging or sun damage. Over time, your lower lids can sag so much that they reveal the white of your eye under your iris, a look that’s unflattering and aging. Fat also collects in the delicate under-eye area, creating puffy looking bags that make you seem perpetually tired, no matter how much sleep you’ve gotten.

Dr. Miller reverses age-related changes in the lower lid with a lower blepharoplasty. These incisions can be camouflaged under your lower lashes or inside your lid, then Dr. Miller tightens the underlying muscles, removes excess fat, tightens your skin, and trims away redundant skin.

Free from bags, sags, and dark circles you look youthful and rested. Many women and men elect to have both upper and lower blepharoplasty at the same time, with or without a brow lift.

Do you want to change your look?

If you have a monolid and desire a double eyelid, talk to Dr. Miller. He’s an expert at natural-looking facial plastic surgeries — including eyelid surgeries — that respect your ethnicity while giving you the improvement you want.

Are you still not sure about surgery?

If you’re still not sure you’re ready for surgery and your eye droop or bagging is minimal, there are a few options. Fillers used in combination with Botox are an alternative that may give temporary improvement to the under eye area. This is a great choice for patients looking for a non-surgical option or can be used in conjunction with surgery to meet your aesthetic goals.

Another non-surgical option Dr. Miller may recommend to revive your eyes is laser therapy. He employs groundbreaking Face-Tite™ radiofrequency(RF)-assisted liposuction technology by InMode to remove excess fat and tighten skin in the facial area.

Face-Tite works by sending pulses of RF energy through a tiny cannula. The energy heats up the fat, killing excess fat cells, while simultaneously initiating a collagen-remodeling process in your skin. Over the next weeks to months, your face tightens and your skin rebuilds itself with new, strong strands of collagen and elasticizing elastin.

Face-Tite can be used as a standalone therapy or in conjunction with facial plastic surgery. In addition to tightening your skin, Face-Tite makes it stronger and more pliable and also aids in the postsurgical healing process.

Either with Face-Tite or on their own, dermal fillers and fat grafts offer yet another way to rejuvenate your eye area. Dr. Miller performs nonsurgical eyelifts and fills tear troughs using state-of-the-art fillers, such as Restylane and fat grafts.

Get “Tite” with These Great Treatments

Skin Tightening Treatment in New YorkWrinkles get a lot of press when it comes to aging, but they’re not the only issue that can make you look older than you really are. In fact, sagging skin is one of the most frustrating signs of aging. This is skin that has lost its elasticity and firmness and has begun to droop and hang as a result. So what can you do about sagging skin? First, you need to understand why it happens. Collagen is a structural protein that gives the skin its firmness and youthful elasticity. As we age, collagen begins to break down and new production also slows. This means that the skin no longer has the support it needs to stay youthful and it begins to sag.

The good news? Your skin isn’t doomed once collagen production slows down. You can encourage new collagen to form by choosing to undergo skin treatments with a plastic surgeon. A series of “Tite” treatments is a great choice for sagging skin, whether you need face, body, or neck skin tightening. Let’s take a look at the options!

Face-Tite

For many people, the face is the most pressing area of concern when it comes to aging. It’s easier to hide sagging skin and other issues in other areas of the body with clothing. But our faces are the first impression people get of us. Face-Tite is a great solution for sagging skin that can help contour the face for a more youthful appearance.

Using radiofrequency energy combined with gentle liposuction, the face is contoured by using small cannulas (tubes) to remove unwanted fat through tiny incisions. It’s a minimally invasive procedure that helps stimulate collagen production for tighter skin, while the liposuction removes any unwanted fatty deposits, allowing the skin to contract. The fat removed is minimal, but the overall results can be dramatic. Patients can look more youthful and refreshed, often gaining enhanced confidence following treatment!

Neck-Tite

Just like Face-Tite, Neck-Tite makes use of radiofrequency-assisted liposuction to tighten the neck and jowls. While sagging in the face is common and noticeable, sagging in the neck can be even more troublesome. Many people develop a “turkey wattle” look that can make them look much older than they really are. Neck-Tite contours the neck and jowls and helps the skin to tighten and contract for a fresh new look.

BodyTite

Stubborn fat and cellulite can be some of the most frustrating problems you can face, as no amount of diet and exercise can help resolve certain problem areas. BodyTite is a great solution for people who just want a sleek physique without the little lumps and bumps that persist, despite a healthy lifestyle.

Radiofrequency assisted liposuction is a gentle procedure that can help create a more toned appearance with little downtime. It also assists with tightening the skin, due to increased collagen production. Good results from liposuction require the skin to retract properly, and the subtle but powerful results of BodyTite accomplish just that.

Tite-FX

Tite-FX uses the same radiofrequency technology as the other procedures, but instead of combining the energy with liposuction, it uses vacuum technology to remove excess fat non-invasively. This treatment is fantastic for people with busy lives because it effectively reduces fat and improves the overall body contours with no downtime. Cellulite is a difficult problem to treat. Yet some patients have noticed a reduction in cellulite after undergoing Tite-FX treatment, which can help boost confidence and self-esteem. This procedure can also be combined with liposuction for even better results.

Gradual, Lasting Improvement

The best part about getting “Tite” is that the results are gradual and long-lasting. You’ll notice a difference right away, of course, due to the contouring of the treated area, but the radiofrequency energy will produce other benefits that will become apparent gradually. You should notice your skin tightening over several months following treatment as new collagen is produced to support the skin. While this won’t last forever, the new collagen is long-lasting and should help slow down the effects of aging for a while. Maintaining a healthy weight and lifestyle is a great way to keep the results looking good for a long time to come!

Choosing the Right Doctor

As with any plastic surgery procedure, choosing the right surgeon is absolutely essential to your success. If you’re interested in the power of radiofrequency-assisted liposuction, you will want to make sure you choose an expert to perform your procedure—someone who has the experience and the artistic eye to make your dream a reality.

There are many excellent plastic surgeons in New York, NY, but Dr. Philip Miller is a seasoned provider of all four “Tite” treatments. He knows how powerful the procedure can be, and he helps patients regain their confidence and self-esteem by helping them choose a treatment plan that’s right for their needs. 5 Myths About Breast Reduction

going-outside-post-facelift-facial surgeryDo a quick Google search, and you’ll quickly realize there’s an overwhelming amount of information available on the subject of breast reduction surgery. Not all of this information is reliable, however. Some pervasive myths surround the procedure, some of which can actually be harmful to prospective patients. If you believe some of these myths, you might delay a surgery that would improve your quality of life, or worse, rush into a surgery before you’re ready. With that in mind, we wanted to bust some of the common myths about breast reductions to help you make the decision that’s right for you.

1. Women should be happy to have large breasts

Many women, and men, are shocked to hear that some women want smaller breasts. But choosing to undergo breast reduction is a personal decision, and a perfectly legitimate one. Some women experience severe back pain from the weight of heavy breasts. In fact, some insurance companies cover breast reduction for exactly this reason. Others find that their large bust is an impediment to exercise or makes it nearly impossible to find clothing that fits. In most cases, the surgery is not cosmetic at all—it’s medically advisable. There are cosmetic benefits as well. The breasts typically look more youthful, firm, and perky after a breast reduction. The bottom line is that it’s not a bad thing to want a smaller bust, and you shouldn’t feel like you have to justify your desires to others.

2. Breast reduction causes an inability to breastfeed

If performed correctly, most patients should have no problem breastfeeding once they have healed from breast reduction surgery. Glandular tissue is removed during the surgery, but as long as the connection between the nipple and core breast tissue is not severed, the procedure should not affect milk production. However, in some breast reduction cases, it is advisable to reposition the nipples, which can affect the ability to breastfeed. There is also always a small risk of complications, which could include loss of the ability to breastfeed. However, this is very rare, especially when the procedure is performed by an expert, board certified plastic surgeon.

3. Weight loss can reduce breast size

Breasts are made up of more than just fatty tissue; glandular tissue is also present. Because of this, weight loss will not provide the kind of relief most breast reduction patients seek. While losing weight can reduce the size of the breasts a little, there’s more to a reduction than just liposuction. Both fat and glandular tissue are carefully removed to create an attractive shape and lift, something that requires the skill of an experienced plastic surgeon. Weight loss can be a good first step, but it can’t replace a breast reduction for significant results.

4. Patients can choose their cup size

As with any plastic surgery, it’s very important to have realistic expectations for breast reduction. While it is possible to aim for a general size range, the procedure is complex, and results cannot be fully predicted before the surgery. A computerized preview can help patients visualize the final results, but surgeons cannot reproduce an exact copy of those previews. Patients need to understand that variations in the surgery and healing affect the final size. It’s best to leave shopping for after the procedure! Unlike breast augmentation patients, breast reduction candidates don’t have implants to choose and can have more variation in the final results.

5. The procedure can be done without scars

All surgery produces some scarring. Depending on the amount of breast tissue to be removed, breast reduction can result in some significant scars. Some patients have scarring extending down from the nipple and all the way under the breasts. While these scars will fade somewhat over time and are easily hidden by a bra or swimsuit, scarring is just part of the breast reduction process.

Talk to a Trusted Surgeon

Because there is so much misinformation available online, you’re better off going directly to the source: a trusted, board certified plastic surgeon. Speaking to a trusted plastic surgeon will allow you to determine your candidacy for the procedure, ask specific questions, and get reliable answers. You will need to consult with each surgeon you are considering anyway, and these meetings are a great way to get to know the surgeons and learn more about the procedure. You should consider your decision very carefully, as breast reduction is permanent.

If you’re ready to get more information about breast reduction and start talking about your goals for the procedure, consider heading to New York, NY for a consultation with Dr. Douglas Steinbrech.
