Botox

Botox -- or Botulinum toxin -- This is not a filler like collagen orRestalyne or Radiesse or Juvader.  Those items are “volume” treatments when there is a wrinkle or depression that needs to be inflated!  . Instead, Botox works by relaxing  the muscles that pull on the skin and form a wrinkle. It is superior to any volume treatment listed below  for improving deep vertical creases between the eyebrows (frown lines), horizontal furrows on the forehead (surprise lines), and crow's feet (squint lines) to the side of the eyes.  It can also be used for lines that surround your lips, lines of either side of your nose, bands or lines on your neck.    As Botox does not cause allergic reactions, no test dose is needed. 

The good news -- the needle used in these injections is finer than a strand of hair and there is no recovery period. You could go out to dinner the same night, looking a little younger than you did that morning!

Volume Enhancers:  Restalyne, Radiesse, Artefill, Juvader.
Facial plastic surgery and chemical or laser peels cannot improve every skin problem.  Neither can Botox.   The crease between your eyebrows, or the furrow formed at the corner of your mouth might be too deep. You might have a small crease between the corner of your mouth and chin, ( the jowls)  which makes you look angry or unhappy all the time. Or maybe crow's feet (squint lines) or vertical lines between your eyebrows (frown lines) are not completely resolved with Botox alone.  . Although certain operations can improve these, you may not be quite ready for the time or expense of plastic surgery. A reasonable alternative might be as simple as “inflating” a depressed or wrinked area. 
There are many different solutions to this problem.  They are collectively called “volume enhancers”.  Some last longer than others, some are better in certain areas of the face than others.  Most are temporary and require periodic treatments, others are permanent.  Which one is right for you?  Well, here is a brief introduction to the products:

 Radiesse: A Longer Lasting Method to Reduce the Appearance of Facial Lines and Folds.  Radiesse is one of the more recent FDA approved products for the treatment of facial “deflation” and to reduce facial wrinkles.  This injectable product offers longer lasting results than other methods currently being used.   Radiesse  will last for a year or so , as opposed to months, and is shown to cause little to no allergic reactions or other side effects that may be elicited with other injectable methods.  Radiesse  consists of calcium hydroxylapatite (a derivative of the human bone) mixed into an easy to use gel form.  In many studies, this product has been proven to remain soft for long periods of time when injected into soft tissue, such as facial tissue.  Radiesse is our preferred treatment of the nasolabial lines- the wrinkle or furrow that forms from the side of the nose to the side of the mouth.  

Artefill.  The most recent FDA approved injectable and “The First to Last” as promoted by the company.  That is, it is the first product for wrinkles that is permanent.  Injected beneath the skin  to plump up any unwanted crease or wrinkle, Artefill is composed of collagen which ultimately dissolves and the permanent polymethylmethacrylate (PMMA) microspheres which promote a natural response of your skin cells ( fibroblasts) to produce collages that encircles the spheres and produces volume enhancement and wrinkle correction. (A  small percentage of the population has an allergy to collagen, so a test dose injected into the forearm is necessary to ensure that it is safe for you.) 

Restalyne and Juvaderm.    These two products are the most commone favorite injectables.  They are made of hyaluronic  acid, a material that is naturally found in your body.  It is purified and then injected.   These treatments last 6 – 8 months, are wonderful for lip enhancement, as well as brow and fine wrinkle solutions.


Volume Enhancement   The theory that aging is sagging skin is naïve.  The fact is, the facial skin not only sags with time, but facial fat deflates, much like a balloon with a pinhole deflates.  This deflation causes the skin to sag because as we get older, the elasticity of the skin, which was excellent when we were younger, is less as we get older.  So, traditionally,  having not recognized the role of deflation, we’ve done facelifts that pull the loose skin.  Sometimes this looks good, sometimes it looks stretched and unnatural.  Two solutions to this problem are Sculptra and Fat injections. 

Sculptra is a product consisting of poly-L-lactic acid which is a constituent of many sutures that are used in surgery.  A bottle of Scultpra is mixed the day before your arrival, your face is anesthestized with a cream, and then it is injected into those areas of your face that may be deflated:  Your cheeks, temple area, Eyebrow area, and the side of your face.  It can even be used to widen the angle of your jaw for a more sculpted face.  Sculptra is like works much like a seed.  You  inject it and it looks great for one day, and then the volume  goes down by the next morning.  Over the next  few weeks and months, your body starts to stimulate collagen volume in resonse to those “seeds” and you will see a gradual and natural inflation of depressed and hollow areas.  Often 2 or 3 treatments are necessary.

Fat Injections.  An alternative to artificial material, fat injections use your own fat to provide volume enhancement on your face.  Fat is “harvested” using a very small cannula from the inside or outside  of your leg or your belly.  The harvested fat is then washed with saline.  It is then reinjected back into those areas of your face that need enhancement and inflation.  There is some controversy as to how long transplanted fat survives.  This is solved by preparing the patient that they may need another treatment in  a few months.  Also, while fat injections are very successful, they should be seen as a complement to current aging face treatments as opposed to a panacea for all aging face problems.  Often, a combination of fat and minor surgical procedures produce the best results. 


Intense Pulse Light Therapy/PROLITE  An easier, more effective treatment for rosacia and other discolorations of the skin Intense Pulse Light Therapy (IPL) or ProLight Facials offer breakthrough technology to help eliminate redness and pigmentation discolorations from sun damage.  IPL can successfully treat sun spots, brown (or aging) spots, red patches, and other pigment disorders of the face and neck with little or no bruising or necessary recovery period.  This treatment is delivered through a gentle light pulse to the skin over several 15 to 30 minute treatments which are administered in three-week intervals. Redness, dark spots, discolorations  and pore size will gradual begin to fade and may disappear completely with each treatment.  And, as treatments progress, skin also becomes smoother and pores appear smaller in size.  Anywhere from four to six treatments are necessary to fully attain a patient's desired look.

Collagen Channeling for Natural Collagen Induction Therapy:
This is a wonderful treatment and consists of rolling a device that contains hundres of needles over your anesthetized face!.  The concept for this treatment came from recognizing that tattooed skin always looked smooth and unwrinked!   So, medical roll cit was invented.  The principle of the Cosmetic Roll-Cit is that the "micro-needles" puncture the outer layer of the skin ( which is uninvolved in collagen induction) , and stimulates the deeper layers of your skin ( which are responsible for collagen creation. ) Furthermore, the needles create temporary passageways to allow topical solutions like vitamin A and Vitamin C to get to the deep skin layer.  Most  lotions that you put on your face are unable to get to the deeper layers of your skin because the superficial layer stops it.   . Once this layer has been breached, the active ingredients in the skin care products can reach into the depths of the skin about 10,000 times more effectively than by simply applying the products topically. The higher the levels of vitamin A and vitamin C that can penetrate into the lower layers of the epidermis, the more collagen can be made. Therefore allowing natural collagenesis to occur.
